dc.description.resumo | Introdução: A hipertensão arterial pulmonar (HAP) é o aumento da resistência vascular da pequena circulação e consequentemente sobrecarga do ventrículo direito. É uma doença grave, progressiva e fatal, que acarreta importante limitação funcional, e dispnéia aos esforços. Os testes de campo de avaliação da capacidade funcional são alternativos ao teste cardiopulmonar (TCP) por serem mais simples e de menor custo. O mais utilizado dentre eles é o Teste de Caminhada de 6 Minutos (TC6), porém existem evidências de que TC6 não reflete a real tolerância aos esforços dos indivíduos com HP. Desenho do estudo: Transversal. Objetivos: Comparar o desempenho de 3 testes de campo com o teste cardiopulmonar na avaliação da capacidade funcional de indivíduos com HAP. Métodos e Resultados: 20 pacientes (49,8 ± 14,6 anos de idade), classe funcional WHO II e III e com média de pressão arterial pulmonar média de: 45 mmHg, foram submetidos aos testes de campo: TC6, Incremental Shuttle Walk Test (SWTi) e Endurance Shuttle Walk Test (SWTe) e também ao TCP em dias diferentes e não consecutivos. O SWTi atingiu níveis máximos de esforço e não apresentou diferenças significativas em nenhuma variável avaliada com o TCP. Já o TC6 e SWTe demonstraram ser testes submáximos. Conclusão: O SWTi é uma alternativa eficaz e segura ao TCP com a vantagem de ser simples e de baixo custo, capaz de reproduzir a capacidade funcional dos indivíduos com HAP. | por |
dc.description.abstract | Background: Pulmonary arterial hypertension (PAH) is an increase in pulmonary vascular resistance and consequently overload of the right ventricle. It is a serious, progressive and fatal disease, which leads to reduced functional capacity, and dyspnea. Functional capacity evaluation field tests are alternatives to cardiopulmonary test (CPET) to be simpler and less costly. The most common among them is the Six Minute Walk Test (6MWT), but there is evidence that 6MWT does not reflect the real tolerance to the efforts of individuals with PAH. Study design: Cross over. Objectives: To compare the performance of three field tests with the cardiopulmonary exercise test in the assessing the functional capacity of patients with PAH. Methods and Results: 20 volunteers (49.8 ± 14,6 years old), WHO functional class II and III and a mean pulmonary artery pressure: 45 mmHg, were subjected to field tests: 6MWT, Incremental Shuttle Walk Test (SWTi) and Endurance Shuttle Walk Test (SWTe) and also to CPET in different, non-consecutive days. The SWTi reached maximum levels of effort and no significant differences in any variable evaluated with CPET. But the 6MWT and SWTe proved submaximal tests. Conclusion: The SWTi is an effective and safe alternative to CPET with the advantage of being simple and inexpensive, capable of reproducing the functional status of patients with PAH. | eng |
